1.US passport 

2.  If ever held Indian passport, Indian passport and Naturalization certificate 

3.  Proof of address (Lease copy, driver's license, or utility bill)

4.  2 passport size photographs          (2x2, white background). We do take pictures in our office if you would like our service.

5.  Parents' Passports and Marriage Certificate 

   - If holding Indian passport, then proof of valid status

   - If US passport holders, we also need naturalization certificate required as well 

6. Birth Certificate if born outside of India(TO BE APOSTILED)

7. If Marriage issued  outside India(Marriage Certificate needs to be Apostlied)

VisaCare Service Charges: $150 per application (If remote service, $250)
